OakMed Home Nursing & Rehabilitation is looking to add PT, OT to our nursing team in the Grand Rapids Area that will be providing care to our patients in their home. The perfect candidate will be professional and compassionate to provide meaningful, one-on-one care to our patients. This is a great opportunity to gain experience, while having a flexible schedule.

Responsibilities
- Conduct patient assessments and develop individualized care plans based on patient needs.
- Utilize EMR and EHR systems to maintain accurate patient records and documentation.
- Administer basic life support as needed and ensure patient safety at all times.
- Collaborate with healthcare team members to coordinate patient care and ensure optimal outcomes.
- Educate patients and their families on health management and self-care techniques.
- Demonstrate proficiency in airway management and provide specialized care for patients with unique needs.
- Adhere to all regulatory requirements and standards of practice in home care nursing.
- Maintain confidentiality of patient information and uphold ethical standards in all interactions.
Job Type: Part-time
Pay: $45.00 - $75.00 per hour
Expected hours: 10 – 20 per week
